In making a booking with Inside Matters, I am agreeing to this waiver, I acknowledge that I have read and agree to the Infrared Sauna and Red Light Therapy session information and conditions outlined below. I am aware of any health concerns or medical conditions that may impact my ability to use the Infrared Sauna safely and have discussed these with my doctor.

 

Infrared sauna and/or red light therapy should not be used as a replacement for medical treatment from a licensed physician or other healthcare provider. I have been informed that the infrared sauna and red light therapy are generally safe.

If my health status changes at any time, I will discuss this with the Inside Matters team to ensure treatment is still suitable for me. 

I intend this consent form to cover all red light therapy and sauna treatments at Inside Matters and understand all fees for services are due at the time of service and can be paid online at the time of booking.

 

I will be charged 50% for appointments that are canceled with less than 24 hours’ notice and the full fee for cancellations within 2 hours or no-shows. No refunds will be given after the services have been used. If there is a problem with your service please let us know and we will be happy to discuss this with you. Occasionally you may be emailed wellbeing information and promotional deals from Inside Matters. You can subscribe from these at any time.
